Work-Study Checklist
Authorization to begin working

The following forms are required before the student can begin working.  Once the forms are completed in its entirely the student will receive an authorization form to start work.  A copy of the authorization form should be given to the supervisor for their records.

• Authorization to Apply Funds
• Confidentiality Agreement
• Form NC-4
• Form W-4
• Employment Eligibility Verification (Form I-9)
• Copy of Driver's License and Social Security Card
• Student Employment Orientation Checklist
